Megumi Fushiguro was born to Toji Zenin and an unknown woman in the Jujutsu Kaisen. As for why he carries the Fushiguro name and not Zenin, it's because Toji took his wife's last name, aka Fushiguro, because he didn't want to be associated with the Zenin clan.

Gege Akutami mentions that Toji reverted to his old troubled personality because his wife died sometime after Megumi was born. As a result, Toji did not step up to take on the father role and sold Megumi to the Zenin Clan as he became the new Ten Shadows Technique user in this era.
